Transition Navigation

Choosing Tomorrow's Path.png

A clearer path from school to what’s next.

As children with disabilities move into their teenage years, the focus of educational planning begins to expand. The question is no longer only, “What does my child need to succeed in school?” It becomes, “What will my child need to build a meaningful adult life?”

Education or training. Employment. Independent living. Transportation. Healthcare. Financial and legal considerations. Community resources. Self-advocacy. Suddenly, families are navigating systems and decisions that extend far beyond the IEP.

Transition planning is how we start connecting those dots before your child leaves the school system.

Until children with disabilities reach the age of 14, educational planning generally happens at school through an IEP or 504 Plan and your child's educational programming is contained to the educational environment described by the established plans.

It seems like, all of the sudden, your child turns 14 and it's time to start planning for life outside of the public education system. It feels like the wild, wild west.

What are their goals? What are your goals for your child? In seven years or less, you and your child will have to figure it out without the framework you've been used to.

Who, what, where, when and how? Oh my!
